All new and returning players are asked to complete this Player/Parent Contact form for the 2024 Season. This information is for the private use of the Booster Club to help provide communication, schedules and season updates as well as any dietary/allergy needs as we work to plan meals and events throughout the season. Parents can complete this form on their players behalf. Thanks for helping us connect!

This fee helps us cover expenses for equipment, tournament lunches, court improvements, senior gifts etc.
